The Polyvagal Ladder: Understanding Safety, Threat, and Shutdown

There is a specific kind of stuck that does not look like panic. Your heart is not racing. Your thoughts have not sped up. If anything, everything has gone quiet and a little far away, like watching the argument from behind glass. You know you should say something. Nothing comes.
Most people have a working idea of two nervous system states: calm, and not calm. Fight-or-flight gets all the airtime, so "not calm" tends to get filed under anxiety, adrenaline, a racing pulse. But there is a third state, and it does not look anything like panic. Stephen Porges, the researcher behind Polyvagal Theory, mapped it decades ago, and it explains a specific kind of stuck that a lot of stress advice completely misses.
Three rungs, not two
Porges built Polyvagal Theory around the idea that the autonomic nervous system did not evolve as a single dial running from calm to stressed. It evolved in layers, each one older than the last, and each one still fully wired into your body today. Picture a ladder with three rungs.
Ventral vagal: safe and social
Face relaxed, voice with natural range, breathing easy. This is the state that lets you have a real conversation, digest a meal properly, and read someone else's tone without bracing for a threat. It is also the newest state, evolutionarily, and the first one to go offline under stress.
Sympathetic: fight or flight
Heart rate up, muscles primed, attention narrowed onto whatever feels threatening. This is the state most stress advice is written for, which is part of why "just take a deep breath" works for some people and does nothing for others.
Dorsal vagal: shutdown
The oldest circuit, shared with reptiles. When fighting or fleeing is not possible, or the threat is too big or too prolonged, the body conserves energy instead: flat affect, numbness, a sense of distance from your own body. This is freeze, not calm.
The ladder metaphor, popularised by therapist Deb Dana in her work applying Porges's research to clinical practice, is useful because it captures something real: these are not three unrelated moods. They are ordered. Under mild stress, most people drop from ventral vagal into sympathetic. Under stress that feels inescapable, prolonged, or overwhelming, the system can drop further, past fight-or-flight, into dorsal vagal shutdown.
Neuroception: the body decides first
Porges coined the term neuroception for something that happens well below conscious thought: your nervous system is constantly scanning faces, tones of voice, room layout, and internal body signals, and sorting all of it into safe, dangerous, or life-threatening, before you have formed a single word about what is happening.
This is why "just calm down" so often fails. It is addressed to the part of you that talks. Neuroception is not that part. It is closer to a smoke detector than a debate. A smoke detector does not wait for your opinion on whether the smoke is dangerous. Which means the way back up the ladder has to speak the smoke detector's language: physical, immediate, and largely non-verbal. This is also why breath, a physical signal the body can read directly through the vagus nerve, tends to work faster than reasoning with yourself.
From experience
For a long time I only had a word for one of these states. Anxious, wired, on edge, that one I understood. But there were other moments, usually after something had built up for too long, where I would just go flat. Not sad exactly. Just absent. People would ask if I was okay and I genuinely did not know how to answer, because nothing felt like anything.
Learning that this had a name, and a different mechanism than anxiety, changed what I tried when it happened. Slow breathing had always been my default reset. It turns out that was the wrong tool for that particular state, and knowing that made more difference than the breathing technique itself.
Why the same breath does not work everywhere
This is the part most breathwork content skips, and it matters. Coherent Breathing at a slow, resonant pace is genuinely effective for climbing from sympathetic back up to ventral vagal. It downregulates a system that is revved too high. But dorsal vagal shutdown is not a revved system. It is a system that has already conserved down. Applying more downregulation to a state that is already shut down does not help, and for some people it can deepen the very numbness they are trying to get out of.
On the middle rung
What helps: slowing downExtended exhales and resonant-frequency pacing (see Coherent Breathing 101) engage the vagal brake and pull a mobilised system back toward safety.
On the bottom rung
What helps: gentle activation firstMovement, upbeat rhythm, an alerting sound, or a firmer physical sensation (cold water, pressure, standing up) before any slow breathing is introduced. The goal is to come up to sympathetic first, then descend from there.
Kozlowska and colleagues (2015) describe this as part of the wider defense cascade: freeze is not the absence of a stress response, it is a distinct stage of one, with its own physiology. Treating it the same as an anxious, activated state gets the direction of travel backwards.
Matching the tool to the rung

That is part of what a synchronised breathing circle is actually offering: not just a shared rhythm, but a room full of nervous systems signalling safety to each other without anyone having to say a word.
Climbing the ladder is rarely a straight shot from shutdown to calm. Most people pass back through fight-or-flight on the way, and that is not a setback. That is the route.
Knowing which rung you are on
Next time something knocks you off balance, try noticing the physical signature before naming the emotion. A racing heart and tight chest point toward the middle rung. A sense of distance, flatness, or heaviness that will not lift points toward the bottom one. The first question is not "how do I calm down." It is "which direction am I actually trying to travel."
